- NCR/Carbonless Form 2 part (White, Canary)
- NCR/Carbonless Form 3 part (White, Canary, Pink)
- Glued on the short edge
- Full and solid color backgrounds not recommended
- Light ink coverage highly recommended
- At least 0.25" of space on the glued edge (short edge) should be free of ink
